Dynamic form testing

How do i do a dynamic form testing (i mean... the form will have to accept values depends on the fields available in a table... and the table fileds may vary by name or increment fields).
for eg : the first day.. the table (so the form) will have fields studentNo, stusentName, class and the values need to be entered/acceted thru a form....and the next day the table will have another field called studentGrade + studenttotalMarks (so the form will have these values also).
for me.. i want to check the fields of the table and assume that these many fields would be there in the form... and to accept the values/texts... how does it possible?
